Prior to your visit, it is your responsibility to know your healthcare coverage and coverage benefits.

You will need to verify your insurance benefits.

Please call your insurance or go to your insurance portal to verify your benefits.

We have provided a link to our form to help you with that process below.

Prior to your appointment, you will need to call your insurance company to verify the specific benefits for which you are covered in and out of network coverage (naturopathic care, provider you can see, certain codes that are covered, preferred lab draw sites and imaging), your copay, co-insurance and determine how your deductible is applied (if applicable).

Our doctors are contracted with most commercial plans but we recommend verifying that your doctor is contracted with your specific plan as some individual and family plans may not be covered.

Out-of-Network:

If you choose a provider who is out of network with your insurance plan, we can bill your out of network insurance benefits for you. It is important to know that you are responsible for the entire amount billed if it goes to your deductible and we bill at a higher rate to insurance than we do for self-pay patients. You also are given the opportunity to pay out of pocket (with a discount) at the time of service and then you can self-bill your insurance.

Medicare:

If you have Medicare, we ask that you call our office to discuss insurance options. We cannot bill Medicare directly, but may be able to bill insurance if you have a Medicare Advantage plan that we are contracted with. It is always best to check with your insurance company but we will also share what we know with you when you call.

Depending on your insurance coverage, you will be responsible for:

  • Co-Payment (paid at time of visit)
  • Deductible, if applicable
  • Co-Insurance, if applicable
  • Any non-covered services
  • Full payment, if no insurance coverage.
  • A 20% discount is offered, if you do not have insurance for the services provided.

Health Savings Accounts:

We will be happy to provide you with an itemized invoice for self-submission as well as provide documentation for your health savings accounts (HSA) or flex spending accounts (FSA).
